How to Use This Template

  1. Write your goal in one clear, measurable sentence. Vague goals like ‘get fit’ will not work here, be specific.
  2. Fill in the Why It Matters field honestly. This is what you read on tough weeks. Make it personal.
  3. Define one meaningful milestone for each of the three months. Month 3’s milestone should be your goal complete.
  4. Print three copies, one per month. Check off each weekly box at your regular weekly review and fill in the monthly notes row at the end of each 4-week sprint.
